What to bring to your first appointment:

• Copay (if applicable)
• Immunization record (if available)

Transfer Medical Records

If your child previously received care from another provider at another clinic or office, his/her records are not automatically transferred to our office.

Please fill a request for medical records and either send it to us so we can submit it or you can also request it yourself. Your provider may want the records before an appointment so do it as soon as you can.
